Medical Clearance
When should I call to make my appointment?
You should call our office 2 weeks before your surgery date to make an appointment.
What do you need for your appointment?
Paperwork from your surgeon that lists the rests that are being requested.
Please have your surgeons office fax it to our office at least 2 days prior to your appointment. Please make sure the paperwork includes your name, dob and surgery date.
What provider do I make the appointment with?
You will ask to make the appointment with Dr. Quaglia he is the main provider that is in the office Monday – Friday.
How long do the forms and labs take to get to you surgeon's office?
The clearance forms and labs take a week to get back to Dr. Quaglia to approve to send out to your surgeon’s office.
